Children With Auditory Neuropathy Spectrum Disorder (ANSD)

چکیده
In 1991, Arnold Starr and colleagues described an 11-year-old child with the paradoxic findings of absent auditory brainstem responses in the presence of normal cochlear microphonics (CMs) and otoacoustic emissions (OAEs). The child demonstrated severely impaired speech understanding despite a mild loss of hearing sensitivity. Further evaluation showed impaired temporal processing as exhibited by abnormal findings on a battery of psychophysical tests that included gap detection and binaural masking level differences. In 1996, Starr and colleagues reported on a group of 10 children and adults with similar findings (Starr, Picton, Sininger, Hood, & Berlin, 1996). Although the patients presented with no apparent neurologic involvement when their hearing impairments were first identified , eight of the ten showed later evidence of other peripheral neuropathies. Starr and colleagues coined the term " auditory neuropathy " (AN) to describe patients whose hearing impairment was attributed to " neurop-athy of the auditory nerve. " Since the initial report by Starr and colleagues it has become clear that individuals diagnosed with AN are a heterogeneous group even though they may exhibit common audiologic findings. Some individuals who exhibit the AN pattern may have a congenital form of the disorder resulting from a genetic mutation or pre/perinatal causes, whereas others may have a later onset form of the disorder associated with other peripheral neuropathies. Speculation regarding the underlying mechanisms of AN includes selective inner hair cell loss, a synaptic or myelinization disorder, or an auditory nerve disorder with other peripheral neu-ropathies

The Effect of Amplification on Cortical Synchrony in Children with Auditory Neuropathy Spectrum Disorder (ANSD)
! The benefit of hearing aids as a clinical intervention strategy for children with Auditory Neuropathy Spectrum Disorder (ANSD) remains controversial. The goal of this study was to determine whether amplification through hearing aid use increases cortical phase synchrony in children with ANSD. Using inter-trial coherence (ITC) of the EEG signal as our measure of cortical phase-locking, we exam...

UNLABELLED OBJECTIVE. Auditory neuropathy spectrum disorder (ANSD) affects approximately 10% of patients with sensorineural hearing loss. While many studies report abnormalities at the level of the cochlea, auditory nerve, and brainstem in children with ANSD, much less is known about their cortical development. We examined central auditory maturation in 21 children with ANSD. DESIGN. Morphology...

Background. Auditory neuropathy spectrum disorder (ANSD) has been reported in up to 10% of all children with hearing loss. It is widely accepted that it can be difficult to identify and manage children with ANSD through typical clinical procedures, in comparison to children with sensorineural hearing loss (SNHL). An important limitation in managing children with ANSD is that behavioral pure-ton...

PURPOSE This review summarizes current evidence related to the audiologic management of children with auditory neuropathy spectrum disorder (ANSD). METHOD A systematic search of the literature was conducted in 25 electronic databases (e.g., PubMed, CINAHL, and ERIC) using key words such as auditory neuropathy, auditory neuropathy spectrum disorder, auditory neuropathy/dyssynchrony, and hearin...
متن کاملAn Update on Auditory Neuropathy Spectrum Disorder in Children
Auditory neuropathy spectrum disorder (ANSD) is a form of hearing impairment in which cochlear (outer hair cell) function appears normal but afferent neural activity in the auditory pathway is disordered. It is now over 20 years since the first paediatric ANSD cases appeared in the literature and over 15 years since the disorder was identified in infant populations. In the ensuing period our un...
متن کاملAmplification Options in Children with Auditory Neuropathy Spectrum Disorder
Auditory neuropathy spectrum disorder (ANSD) is a specific form of hearing loss characterized by the presence of normal or near-normal Otoacoustic Emissions (OAEs) but the absence of normal middle ear muscle reflex and severely abnormal or completely absent auditory brainstem response (ABRs). Amplification is considered a dilemma in such disorder with a lot of debates. This article will review ...
